To mail in orders, please write your address, phone number, and email on a sheet of paper. Include item name and quantity, and add in shipping charges. Connecticut residents must also include 6.35% sales tax on all items. Include payment and mail to the address below. Alternately, you may use the online order system and choose the check or money order option, print out your order page, and mail it in with payment to:

Automatic Monthly Shipments
To establish a "standing" order for automatic monthly shipment, please call or fax in 203.302.1991 the following information: Item(s), quantity and size if applicable, billing address as it appears on your credit card statement, shipping address if different, and credit card number with expiration date, or Paypal email address. Please indicate the day of the month that you would like your order to ship out, keeping in mind transit times.
Your credit card information will be securely kept on file, and your card will be charged before your "standing" order ships every month on the specified day. However, if the specified day falls on a holiday or weekend, your order will ship on the closest business day BEFORE the requested date, so as to avoid delayed shipments and subsequent missed doses.
A declined credit card will delay shipment of your order. A second card may be called in to be kept on file should the primary card be declined. Please keep your credit card information updated.
Those wishing to use Paypal for payment will receive a request for payment one to two days before their order is planned to ship, and will ship once payment is received.
"Standing" orders may be cancelled, added to or altered anytime between shipments. All that is required is a call or email (203.302.1991, firstname.lastname@example.org ) at least 3 days before the actual shipping day, which may be earlier than your requested date should a holiday or weekend fall on that day. Please make sure that you receive a confirmation of the change or addition. If you have not received one within 24 hours, please submit it again.
We will do our very best to honor all last minute changes or additions, but can not be held responsible for late notification. In such cases, additions will be sent out separately and charged shipping costs, but only with your prior approval. Otherwise, additions will be held and shipped with the following months shipment. Item changes and address changes to an already shipped order will incur regular shipping charges. Again, please try to give several days notice.
We realize that regular shipments may produce a backlog of product and thus allow for 2 months to be skipped in a calendar year without affecting continuous status of automatic shipments. Please let us know of an impending skipped order at least 3 days before your actual shipping date.
